Integrated Health Homes “expand the traditional medical home models to build linkages to other community and social supports, and to enhance coordination of medical and behavioral healthcare, in keeping with the needs of persons with multiple chronic illnesses.” – Centers for Medicare and Medicaid Services
We recently helped a woman, with a history of depression, that had stopped all of her high blood pressure medications and insulin for over one year. She had been seen in the emergency room two weeks prior for minor problem, and was advised to seek help for her medical condition. She came to Hillcrest and was put on insulin for very high uncontrolled glucose, blood pressure medication for high blood pressure, and referred to an endocrinologist. She also needed medications for high cholesterol. We individualized a plan of care for her that includes reminder calls to keep appointments and diabetic education. She is very appreciative of our help for her brain health and physical problem, and without Hillcrest, she indicated that none of this would have been addressed.
