Whether you’re adjusting to life on your own, managing a health condition, or reconnecting with your community, we meet you where you are—with care, respect, and encouragement.
You’ll be matched with a Community Integration Coach who listens to your goals and supports you, step by step.
Our program offers flexible in-home and community-based services focused on daily living, health, and connection.
We help you manage medications, build life skills, and stay housed—while supporting your emotional and mental wellness.
This program is open to individuals who:
Are 18 or older
Diagnosis of Serious Mental Illness
Have Had a Brain Injury
Have Intellectual Disabilities
Qualify for Medicaid, Regional Funding, or Private Pay
We help with cooking, cleaning, budgeting, and other life skills needed to live independently.
We offer education about medications and help you coordinate mental and physical health care.
You’ll receive support managing stress, navigating relationships, and building emotional resilience.
We help connect you to transportation, housing, education, benefits, and job opportunities.
We create plans to help you stay safe, avoid crisis, and reduce hospital or emergency visits.
Everything we do is guided by your pace, your preferences, and your goals for daily life.
We accept Medicaid, regional funding, and private pay. Grant funding may be available.
