PATH (Projects for Assistance in Transition from Homelessness) is a federally funded outreach program that connects individuals experiencing homelessness—or at risk of becoming homeless—with the services and support they need.
Our goal is to listen, walk beside you, and connect you with resources that promote long-term stability and hope.
This program is for adults (18 and older):

Live in Dubuque or Dubuque County
Seeking Support & Resources
We listen to your story and connect you to services that fit your specific needs.
Our team helps you access housing, healthcare, mental health care, and community-based support.
We partner with shelters and housing programs to ensure coordinated, wraparound care.
Advocates walk beside you to help navigate complex systems with clarity and compassion.
When available, we offer help with rent, deposits, utilities, and other basic living costs.
We support access to transportation, medication, and emergency funding through resources like the Envision Grant.
Available to eligible individuals, no insurance required.
